Nurse Practitioner

Job Summary

The Nurse Practitioner is responsible for providing patient centered, professional and compassionate health care to adult clients, with a focus on underserved, uninsured populations. This role supervises the Nurse Supervisor and works in conjunction with the Medical Director to ensure that high-quality standards are met in the delivery of health care services. This position is part of the leadership team of MKHC, which focuses on enhancing operational efficiency and care delivery at MKHC.

Part time: 24-32 hours a week

​

Accountabilities

Direct Patient Care 80%

· Provide primary health care services for clients with acute and chronic illness according to evidence based standards of care.

· Perform comprehensive physical assessments, diagnose health conditions, develop treatment plans, prescribe medications, and refer patients as needed.

· Delegate tasks effectively to medical assistants and other team members to support the patient care plan.

· Ensure excellent access to care for assigned patients, including via phone, email, and in-person consultations.

· Conduct follow-up communications regarding lab results, consultations, and test outcomes to ensure continuity of care.

Quality Improvement Activities 10%

· Provide guidance on continuous quality assessment and performance improvement activities.

· Review data to inform the quality improvement activities.

· Maintain high patient experience scores by ensuring compassionate, responsive, and timely care.

· Assist in the formulation and implementation of clinical policies that enhance the quality of care delivered.

· Consult regarding medication procurement and the acquisition of cost-effective medication options for uninsured patients. Implement health education and disease prevention activities.

 

Leadership 10%

· Works closely with executive and MKHC leadership to provide input on clinic management, processes and future goals.

· Collaborate with MKHC leadership to inform and improve clinic operations, policies, and long-term goals.

· Contribute to the development and streamlining of clinical processes, ensuring alignment with MKHC's mission and commitment to serving uninsured patients.

Supervisory/Management Responsibilities

This position supervises the Nurse Supervisor and reports to the Medical Director.

 

Minimum Requirements

Advanced Degree in Nursing with current Maryland NP License

2+ years of clinical NP experience in a primary care or urgent care setting.

Board certification in Family Medicine (FNP) or Adult-Gerontology (AGNP)

 

Knowledge, Skills or Abilities

· Clinical Knowledge: In-depth knowledge of primary care practices, including the ability to manage a range of acute and chronic conditions, and the ability to deliver high-quality, patient-centered care.

· Cultural Competence: Ability to effectively communicate and provide care for diverse populations, particularly underserved and uninsured individuals. Understanding of socioeconomic and cultural barriers to healthcare access and strategies to address them.

· Patient Advocacy: Strong commitment to patient advocacy, ensuring that all patients, regardless of their insurance status, receive equitable care.

· Care Coordination: Ability to coordinate care with multiple providers, ensuring continuity and consistency in treatment, particularly for complex or vulnerable patients.

· Leadership and Supervision: Ability to lead, mentor, and support a multidisciplinary team, including supervising nurse supervisor.

· Medication Management: Knowledge of cost-effective medication options, particularly for uninsured patients, and ability to provide counseling on medication adherence.

· Communication: Excellent verbal and written communication skills, with the ability to communicate complex medical information in a clear and compassionate manner.

· Technology Proficiency: Proficiency in electronic health records (EHR) systems and other clinic technologies.

· Quality Improvement: Knowledge and experience with quality improvement initiatives, data collection, and performance metrics.

· Community Engagement: Ability to engage with the local community to enhance outreach efforts, improve patient care, and promote preventive health practices.

 

Preferred Skills:

· Experience in a non-profit or safety-net healthcare setting.

· Bilingual (Spanish or other languages)
